2K a year JOB SUMMARY 1.
Administers skilled nursing care to patients requiring intermittent nursing services according to the physician's orders and under the direction and supervision of the Director of Nurses.
2.
Teaches and supervises patients and their families, caregivers, and significant others regarding issues relating to the patient's health and well being.
3.
Coordinates all aspects of care for their assigned patients.
PERFORMANCE RESPONSIBILITIES 1.
Complete holistic assesment including physical, emotional, social, and spiritual status.
2.
Coordinates total patient care with all disciplines involved.
3.
Assesses and supervises Home Health Aide care at least every two weeks.
Revise Aide care plan as required.
4.
Carries out prescribed regimens and procedures and assesses outcomes.
5.
Assess patient and report to DON, updates and changes in condition, efficiency of medications, and response to treatment.
6.
Develops interventions aimed at assisting the patient to meet own needs.
7.
Instruct, counsels and supervises patient and caregivers regarding patient care.
8.
Revises Nursing Care Plan as required.
9.
Completes admission paperwork within forty-eight hours of the admission.
10.
Participates in in-service education.
11.
Attends in-service education programs, weekly case conference and monthly staff meetings.
12.
Turns in daily visit records and nursing notes every twenty-four hours of the admission.
13.
Reports directly to DON.
14.
The RN furnishes skilled nursing services by or under the supervision of a Registered Nurse in accordance with the plan of care.
15.
Standard duties of the Registered Nurse.
16.
The Registered Nurse makes the initial evaluation visit.
17.
Regularly re-evaluates the patient's nursing.
18.
Furnishes those services requiring substantial and specialized nursing skill.
19.
Initiates the Plan of Care and nesessary revisions.
20.
Initiates appropriate preventive and rehabilitative nursing procedures.
21.
Performs supervision visits on LPN's every 30 days.
22.
Assists and participates in QA/QI activities.
23.
Prepares clinical and progress notes, coordinates services, informs the DON and other personnel of changes in the patient's condition and needs.
24.
Accepts on-call as scheduled.
25.
