Dietitian - Home Telehealth Weight Managment

Major responsibilities of the incumbent include, but are not limited to:
collaboration with the HT team, screen and enroll Veterans, plan strategies to manage and deliver nutritional aspects of the Home Telehealth Weight - Management Program; develop nutritional care and education; provide quality assurance evaluations; engage in professional development and provide motivational education about Home Telehealth-Weight Management Programs. The increasing complexity and specialization in nutrition requires that the clinical dietitian have expertise in a wide range of disease entities, treatment modalities/Medical Nutrition Therapy and drug/nutrient interactions. The HT-WM Dietitianrequires master of, and skill in applying, the theories and practices of dietetics in accordance with the guidance set forth in Nutrition and Food Services policies and procedures and VA regulations governing the practice of dietetics. The incumbent will provide advice and program consultation on nutrition and weight management programs; develops new approaches and apply current scientific advances; and maintain effective working relationships with public health administrators and different community resource agencies that aid in helping the Veteran change their nutrition behaviors to improve their weight. Duties included for the Inpatient Dietitian but are not limited to:
Assesses nutritional, behavioral and physical activity needs of individuals through analysis of patient histories and the MOVE!11 Questionnaire, screens Veterans for a home telehealth device and assigns the most appropriate device to meet needs of Veteran/caregiver. Guides patient to an understanding of the nutritional aspects of the patient's care. Modifies education techniques based on the patient's individual social, psychological and cultural profile. Reviews expectations with Veteran/caregiver on using the home telehealth program for 82 daily communications that support patients in their self-management of weight during a 90-day period. Assesses biochemical and anthropometric findings, obtains history of dietary intake, and evaluates clinical factors to consider complex health requirements imposed by the patient's specific disease and weight management treatment plan. Monitors patients' weekly weight status and daily answers to weight reduction disease management questions and follows-up when there are out-of-range alerts. Monitors medications, treatment modalities and progress towards goals and intervenes frequently to respond to complicated disease state. Reviews current lab tests to assess nutritional laboratory parameters and monitors weight management outcome measures. Synthesizes data to determine nutritional and weight loss consequences on the system and thus evaluate patient's projected response to treatment. Develops and assesses effectiveness of general and specialized diets and weight management care plans. Develops care plans for severely nutritionally and medically compromised patients. Implements weight management care plans which reflect integration of various disease entities affecting the patient. Develops or identifies specialized feeding programs for morbidly obese patient conditions to enhance weight management outcomes. Assesses compliance by the patient/family caregiver. Completes weekly tracking reports of weights using Home Telehealth data Work Schedule:

The individual must have earned a bachelor's degree from a U.S. regionally accredited college or university or foreign equivalent and fulfilled each of the following criteria:
Completed a didactic program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND), formerly known as the Commission on Accreditation for Dietetic Education (CADE). Completed an ACEND accredited or approved supervised practice program. Supervised practice programs are post-baccalaureate degree programs that provide supervised practice experiences which meet the eligibility requirements and accreditation standards of ACEND, formally known as CADE. Completion of a coordinated program in dietetics fulfills the requirements of a supervised practice program. Time spent in a dietetic internship or supervised practice program does not qualify as creditable experience. Registration/Certification. All applicants must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA). Advanced Level Practice Dietitians must possess a minimum of one advanced practice credential relevant to the area of practice, (e.g. Certified Nutrition Support Clinician (CNSC), Board Certified Specialist in Renal Nutrition (CSR), Board Certified Specialist in Gerontological Nutrition (CSG), Certified Diabetes Educator (CDE), or Board Certified Specialist in Oncology Nutrition (CSO)) Grade Determinations:
GS-11 (Full Performance Level) Experience or Education:
At least 1 year of experience as a registered dietitian equivalent to the next lower grade level, or education equivalent to 3 full years of progressively higher level graduate education, or a Ph.D. or equivalent doctoral degree from an accredited university or college in the field of dietetics or closely related field. In addition to the experience above, the candidate must demonstrate the following KSAs:
Ability to develop and maintain a system of internal reviews and direct quality control and performance improvement studies.Knowledge of basic investigative processes and techniques in order to participate in the design of studies, collect date, interpret findings, and translate results into oral and written communications. In addition to the KSAs identified in subparagraph 1 above, candidates for clinical nutrition positions must demonstrate the following KSAs:
Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy for patients with complex medical conditions in order to plan, develop and coordinate, medical nutrition therapy intervention, interdisciplinary care plans and nutrition education activities. Knowledge of complex areas of clinical nutrition including management of critically ill and malnourished patients and ability to apply medical nutrition therapy to patients with multiple co-morbidities. Ability to act as a mentor/consultant for health care providers including registered dietitians, dietetic interns, nurses, physicians, medical students, and allied health professionals. References:

The position requires intermittent walking and standing and carrying normal amounts of written/reference material on a routine basis. Occasionally, duties may require the movement of audio-visual equipment charts arrangement of classroom areas. Work is sedentary, involving nutritional plans, research and reports.
